Membership Activation:
- Membership benefits activate immediately upon sign-up and first payment.
Membership Duration and Billing:
- Membership is billed monthly at $109 or $1,200 upfront for the year.
- A 12-month commitment is required.
Cancellations and Refunds:
- Memberships can be canceled after the 12-month period.
- Refunds for partial months are not provided.
Payment Details:
- Payments must be made by credit or debit card.
- Late payments must be resolved within 7 days to avoid suspension of benefits.
Scheduling and Appointments:
- Members are responsible for scheduling treatments.
- Availability is subject to scheduling & staff availability.
Discounted Treatment Pricing:
- Botox, Dysport, Xeomin: $12/unit
- Daxxify: $8/unit
Usage Caps and Free Units:
- Membership allows up to 4 treatments annually with a cap of 100 units per treatment.
- 20 free units are provided annually on your membership anniversary and must be used during the membership period.
Discounts and Offers:
- Discounts apply only during the membership period and cannot be combined with other offers.
Member Responsibilities:
- Members must provide accurate personal information and adhere to appointment policies.
Privacy and Data Security:
- Personal information will be kept confidential and used only for membership purposes.
Limitations and Exclusions:
- Benefits are non-transferable and can only be used by the registered member.
- Unused treatments, units, or benefits do not roll over to the next year.
Changes to Terms and Conditions:
- We reserve the right to update membership terms. Members will be notified of changes.
Governing Law:
- These terms and conditions are governed by the laws of the State of Connecticut.
Membership discounts cannot be combined with other promotions or applied to packages unless otherwise specified. A minimum commitment may apply.
